widget (dialog window) position not following scroll

I generated a widget code using your tool, included it in my webpage.
When I scroll down, the small widget "leave feedback" remains visible at the center of the window.
If I click on this widget (after having scrolled down) the dialog that pops-up (DIV#myotziv_box) is displayed on the top of the page (outside of current displayed area).
Please fix position of the dialog for it to show along the small clickable widget.

I tested the issue with Firefox & Chromium, running Ubuntu 12.04.
